Airline Safety Record

In this post, we would like to give information about our groups’ airlines. These are Air Asia, Alaska Air, Delta Aİr, Korean Air.

In this graph, we tried to explain accidents and fatalities of our airlines’ accidents.

* We do not have enough information about Air Asia because The Company is LCC so Air Asia do not release too much information to public.

AirAsia Flight 8501

An apparent mid-air miscommunication between pilot and co-pilot in response to a technical problem caused AirAsia flight 8501 to plunge into the Java Sea on 28 December 2014. It was a scheduled flight from Surabaya, Java, Indonesia to Singapore. The plane wreckage and bodies of the passengers were later pulled from the Java Sea in waters off Kalimantan, the Indonesian part of Borneo island. Seven crew members and 155 passengers were on board.

The findings from an almost year-long investigation by Indonesia’s National Transportation Safety Commission are the first official explanation of what caused the Airbus to crash.

One recommendation was for AirAsia to have a more standardised terminology to avoid such accidents from happening again.

“To AirAsia we recommend that pilots have a standard callout or terminology to communicate so there is no misinterpretation and all pilots are trained in procedures of how to take control,” said Utomo.

The reason the co-pilot had initially taken control of the plane was because the captain was trying to fix a problem with the RTL, an issue that investigators revealed had occurred 23 times on that specific plane since January 2014.

Accident Investigation

Pilots lost their contact just after 42 minutes after take-off. Electrical interruption to the RTL happened three times in the space of 13 minutes, eventually causing the loss of the autopilot function.

With the loss of the autopilot function, the co-pilot was left to fly the plane manually, and it was at that point that there was a miscommunication between the captain and co-pilot, investigators said.

According to information gleaned from two black boxes and a cockpit recording, the pilot instructed the co-pilot to “pull down”, an order that was taken literally, sending the plane soaring up to 38,000 feet. The report said the plane went into a “Prolonged stall condition that was beyond the capability of the crew to recover”. At one point the two men appeared to be pushing their controls in opposite directions, it added.

Regulation after accident

”Human beings, who are almost unique in having the ability to learn from the experience of others, are also remarkable for their apparent disinclination to do so.” ― Douglas Adams, Last Chance to See After an accident learning from the mistakes is the only constructive thing. Accident follow changing in regulations, rules, aircraft structures, and other reasons which led to an accident. As a result of a crash that killed 162 people in Indonesia AirAsia Flight 8501 government tightened regulations on aircraft maintenance and flight crew competence.

Number of new rules regarding permits and safety, including health checks for flight crews and air traffic controllers, have been implemented since the crash. The ministry's air transportation director general, Suprasetyo “We have formed a special team to ensure that every safety action recommended by the KNKT will be completed. We are taking a series of corrective actions as a preventive measure so that the same accident will not happen again in the future,” said after accident. Flight crew to practice flight simulations for the upset recovery of an aircraft every six months, an increase from the previously recommended 12-month timeframe. All 75 Airbus A320 planes registered in Indonesia eximaned after accident.

Even with the advancements in aeronautical technology and weather forecasting, aviation accidents still cannot be avoided. We still hear news about aircraft crashes, loss of control and disappearance due to human errors (e.g. pilot and maintenance error), bad weather, mechanical failure or sabotage. But it’s possible to minimize the loss associated with accidents such as by reducing social panic. According to Aviation Safety Network (ASN), a Netherlands-based online aviation database, the seriousness of aviation accidents can be classified into accident, hijack, incident, other occurrence, unfiled occurrence, write-off and hull-loss. Most aviation accidents are fatal, and involve other political problems, so it always causes huge public responses and concerns. The most direct and immediate effects that we can see after accident is stock price fluctuation. That means that company going to be economic shrinkage. After that downsizing, companies could be able to discharge some employees. In addition, people tend to believe themselves rather then social media, television and exaggerated news. That’s why people choose other airline company after these crises and unfortunately, airline companies going to lost of revenue. Remember that Malasia Airlines Flight 370. This flight was a scheduled international passenger flight that disappeared on 8 March 2014 while flying from Kuala Lumpur International Airport, Malaysia, to its destination, Beijing Capital International Airport in China. The aircraft, a Boeing 777-200ER, last made voice contact with air traffic control, when it was over the South China Sea, less than an hour after takeoff. Another Malaysia event is Flight 17. This flight was a scheduled passenger flight from Amsterdam to Kuala Lumpur that was shot down on 17 July 2014 while flying over eastern Ukraine, killing all 283 passengers and 15 crew on board. These accidents happened in the same year which is 2014, after than this company collapsed.

Aviation accidents cannot be 100% avoided, but we can minimize it and we supposed to be educate the people about accidents. Aviation is one of the safest travelling style and people should be know it. By this way companies couldn’t be bankrupted.
